In 2022, caregivers and covered spouses, on the INTEGRIS Health Medical Plan, each have the opportunity to earn up to a $400 Cash Incentive. To earn the $400 Cash Incentive, select from the below options to help you earn the maximum incentive.
$200 - 2022 INTEGRIS WELLNESS VISIT OR MATERNAL WELLNESS PHYSICIAN VISIT
$200 -CURRENT WITH PREVENTIVE CARE GUIDELINES
The incentives apply to all caregivers and spouses enrolled in the INTEGRIS Health Medical Plan. To be eligible, you must be employed at the time of payout as well as enrolled in the medical plan as of October 1, 2022
All program requirements and verification forms must be met and submitted to Caregiver Wellness by October 31, 2022.
Please be sure to check your Caregiver Wellness Dashboard frequently. Your dashboard reflects the information that Caregiver Wellness has received. If your dashboard is not reflecting correctly, it is your responsibility to notify Caregiver Wellness to rectify the situation, no later than November 30th. Incomplete dashboards will result in the caregivers/covered spouses not receiving the full 2022 cash incentive amount.
Your participation in the Wellness Program is completely voluntary. You have the option to stop participating in the Wellness Program at any time. Any personal medical information that you share in connection with the Wellness Program will be protected and confidential and may not be used for employment purposes.
